As part of the group’s objectives they chose to implement the recommendations of the "Respite Care in Delaware" report: the survey of respite needs and existing services. The DCSC surveyed both families and current providers about respite services in order to gain a better understanding of the state of respite care in Delaware. The study focused on the children’s and families’ experiences from the time the children were born until the children entered kindergarten. The task force, comprised of parents and professionals, began working in the spring of 2002, and their efforts have culminated with this initial report. Because of the extraordinary need for respite care, the task force examined the availability of public and private respite services in Delaware, model programs throughout the nation, and pending national legislation on lifespan respite care. Recommendations based on this research were developed.Item Delaware Technical Initiative: The Alfred I. duPont Hospital for Children Transition Survey Project Final Report(Center for Disabilities Studies, 2007-05) Riddle, Ilka K.This report explores the experiences of young adults with chronic health conditions and disabilities with the adult health care system after transitioning out of pediatric health care. In particular, the report focuses on young adults’ access to primary care physicians (PCP) and specialists, comparison of the pediatric and adult systems, satisfaction with services, and recommendations on how to improve the overall transition process.Item Healthy Delawareans with Disabilities: Health Status Report for Adults with Disabilities(Center for Disabilities Studies and Division of Developmental Disabilities Services, 2007-06) Riddle, Ilka K.This report presents information about the health, wellness, disabilities, secondary health conditions, and lifestyles of adults with disabilities in Delaware. Information from 339 adults with physical, sensory, cognitive and learning disabilities, and mental health conditions was collected from May to December 2006. Project activities focus on two areas: piloting a model for coordinating and providing services for these children; and facilitating the adoption of a system of care approach in the larger child-serving system. This is the final evaluation report and focuses on the key findings of the evaluation of F.A.C.T.’s efforts through the end of state FY2005. This report also presents recommendations for the project and for the Department of Services for Children, Youth, & Their Families system of care effort based on these findings.
