Abstract
This report explores the experiences of young adults with chronic health conditions and disabilities with the adult health care system after transitioning out of pediatric health care. In particular, the report focuses on young adults’ access to primary care physicians (PCP) and specialists, comparison of the pediatric and adult systems, satisfaction with services, and recommendations on how to improve the overall transition process.
