AbstractFalls are a significant health concern for community-dwelling adults over 65, leading to increased morbidity and mortality. Despite the evidence that risk-to-fall assessments reduce falls, data show that primary care practices struggle to assess older adults’ risk of falling. Three commonly cited barriers include limited resources, financial reimbursement, and time to complete assessments. The Centers for Disease Control’s Stopping Elderly Accidents, Death, & Injuries (STEADI) is a risk-to-fall assessment tool that incorporates evidence-based resources and clinical tools for patients and providers in primary care. ☐ This DNP practice change project was guided by Everett Rogers’s Theory of Diffusion of Innovations with the purpose to increase risk-to-fall identification using STEADI in a primary care setting with community-dwelling older adults 65 and over. Data collection included a convenience sample of 116 older adults. A comparison of the practice’s usual screening method during the same period in 2018 was conducted. Data analysis indicates that 51.7% of patients positively screened as increased risk-to-fall, which was an increase from 49.5% compared to the previous year. Using STEADI increased the identification of older adults at risk-to-fall compared to the usual screening method. ☐ Keywords: Adult and Older Adult, Evidence-Base Practiceen_US
